What insurances are accepted at your office? What are our office hours of operation? Office hours are from Monday through Friday 8:00am to 5:00pm. Monday and Friday are set aside for clinic diagnostic testing and hospital procedures. Tuesday through Thursday are clinic patient visit days. We will make every effort possible to accommodate your appointment needs. What is the physical address? 10480 South Main Houston, TX 77025 Directions: From 610 South Loop, exit Main and turn south (outside the loop - away from Reliant Stadium). Stay to the right and exit to the feeder (after Shell Station). Houston Cardiac Association will be on the right side of the road just south of Omera Drive (car wash on corner). We are in a standalone building with a big curved roof. From Sugar Land/Stafford/Missouri City, take 90A east (it turns into Main Street after the big left bend. Exit Bellfort and make a U-turn under the highway. Stay in the right lane. Houston Cardiac Association is located just south of Omera Drive (car wash on corner). All refill requests should go through your pharmacy first. The pharmacist will contact us for authorization. This is the fastest and most complete method of refilling a prescription. The best way to get refills completed quickly is to make sure that your pharmacy has the ability to receive and send prescriptions and prescription requests by email. If so, you should call your pharmacy and ask them to request a refill from us by email. We are able to complete email requests quickly. You may, however, if necessary, contact the office for medication refills. We ask that you do so at least ten days before your refill is necessary. Please allow at least 24-48 hours for the pharmacy to complete the medication refill. Is the parking free? Yes. Houston Cardiac Association is a freestanding building and no fee is required for parking. What do I need to bring with me on the initial visit? You will need to bring a your driver's license or ID, all insurance cards, any payment that is due at the time of service: co-payment, deductible, and self payment. You will also need to bring the referral from your PCP or referring doctor, if required. If you have any medical records that are pertaining to the visit, please bring them also. A current list of your medications (names of medications, strength, and number of times and when taken) will also be needed. Sometimes it is easier to just bring all your medications with you to the office so we get an accurate list. What insurances are accepted at your office? We accept most major insurance companies, but are limited to some HMO plans. Please call your insurance company to verify if Dr. Montgomery (Houston Cardiac Association) is enrolled in your plan or you may feel free to contact the office for further information. |
